基于中高强度身体活动指南的中国儿童青少年每日推荐步数

      要:探析我国儿童青少年每日中高强度身体活动(moderate-to-vigorous physical activityMVPA)时长与步数间的对应关系,开发身体活动指南推荐的每日至少60 min MVPA所对应步数阈值,为我国儿童青少年MVPA评价提供新的实用选择。以上海市、四川省成都市、河南省鹤壁市1 7416~18岁儿童青少年为研究对象,使用三轴加速度计ActiGraph wGT3X-BT连续监测7天身体活动。以年龄、性别为分层变量,随机抽取70%有效佩戴日数据作为方程建立样本,30%有效佩戴日数据作为验证样本;使用一元线性回归模型分析每日MVPA时长和每日步数的关系,得出6~11岁儿童、12~18岁青少年每日60 min MVPA所对应的步数阈值,并通过ROC曲线判别步数阈值的区分效能,最后通过验证样本判别不同阈值区分准确度。结果显示:每日步数与每日MVPA相关性显著(r=0.87P0.001),儿童和青少年达到“每日至少60 min MVPA”的步数分别为10 149~11 319步和9 502~10 660步,ROC曲线结果显示本研究提出的步数阈值的约登指数均高于已有阈值。验证样本中,儿童11 000/天与青少年10 000/天的阈值判断的MVPA达标率与实际达标率的误差最低(-0.2%1.4%),准确率分别为88.5%87.4%。研究认为,我国6~11岁儿童每日11 000步、12~18岁青少年每日10 000步的活动量能够满足世界卫生组织与中国儿童青少年身体活动指南所推荐的“每天至少60分钟MVPA”,可作为评价我国儿童青少年是否满足推荐量的实用标准。

Daily steps recommendation for Chinese children and adolescents based on moderate-to-vigorous physical activity guidelines

Abstract: This study aimed to investigate the relationship between daily moderate-to-vigorous intensity physical activity (MVPA) duration and steps in Chinese children and adolescents, and also to establish step-count thresholds equivalent to the recommended 60 minutes of daily MVPA, so as to provide new and practicable choose for evaluating MVPA of Chinese children and adolescents. A total of 1 741 participants aged from 6 to 18 from the city of Shanghai, Chengdu, and Hebi were continuously monitored for 7 days by using ActiGraph wGT3X-BT accelerometers. Age and sex acted as stratified variables, 70% valid data were randomly selected to develop predictable equations and other 30% of that utilized for testing samples. An univariate linear regression model was used to analyze the relations between daily MVPA and steps, and then steps thresholds for meeting the "≥ 60 min MVPA" recommendation were calculated for children (6~11 years) and adolescents (12~18 years). ROC curves distinguished the discriminative performance of the new and currently used thresholds, and the accuracy of different thresholds was evaluated using the testing samples. Results showed that a significant correlation between daily steps and MVPA (r = 0.87, P< 0.001), and the steps ranges for meeting the MVPA recommendation were 10 149 ~ 11319 steps per day for children and 9 502 ~ 10 660 steps per day for adolescents. In accordance with results from ROC curves in this research, the Youden indices for the new thresholds were higher than those of existing thresholds. As for the testing samples, thresholds of 11 000 steps per day for children and 10 000 steps per day for adolescents showed the smallest error in classifying MVPA compliance (-0.2% and 1.4%, respectively), with accuracy rate of 88.5% and 87.4%, respectively. In conclusion, the activity volume of daily steps of 11 000 for children (6~11 years) and 10 000 for adolescents (12~18 years) in China could effectively meet the WHO and Chinese guideline recommendation of ≥ 60 minutes of MVPA for children and adolescents, and could be served as a practical standard for evaluating whether the children and adolescents in China meet the recommendation volume.
